ONLINE CLASS POLICY & WAIVER: You agree that by participating in online physical exercise or online training activities, you do so entirely at your own risk. You agree that you are voluntarily participating in these activities and using your facilities and premises and assume all risks of injury, illness, or death. By participating in our online classes, you expressly agree to release and discharge the trainer or instructor from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against the trainer or instructor for personal injury or property damage.
LIABILITY RELEASE: We, the staff of Sharpen Up! Dance Team Training, recognize our obligation to make sure our students and their parents are aware of the risks and hazards involved in the sport of dance. By signing this release form, you release Sharpen Up! Dance Team Training and all the employees from all claims on account of any injury or illness which may be sustained by your child attending any class or outside performances associated with Sharpen Up! Dance Team Training. You also affirm you now have, and will continue to carry, proper primary medical, health, hospitalization, and accident insurance, which you consider adequate for protection of both your child and Sharpen Up! Dance Team Training.
MEDIA RELEASE: By signing this form, I grant my permission to Sharpen Up! Dance Team Training, and its owning entities, to use my child's photograph publicly to promote their business. I understand these images may be used in print publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to be by reason of such use.
